This innovative dual case management model will work collaboratively with the young person’s Employment Consultant or Youth Coach to promote improved engagement.
Services include outreach community case management, crisis support to stabilise the young person, advocacy and support with such items as housing, justice, health and substance issues.
Outcomes for your young job seeker will include improved engagement with the jobactive/Transition to Work provider and improved education and employment outcomes. This service is funded by the jobactive/Transition to Work providers.
